TAIPEI, Taiwan -- Taipei City has geared up efforts to tackle a potential H7N9 avian flu outbreak, preparing the isolation rooms at a hospital that helped win the hard-fought battle against SARS a decade ago, health officials said yesterday.

The city municipal hospital's Heping Branch has been designated the center for treating H7N9 patients, because of its facilities and experience in tackling the deadly SARS epidemic in 2003, the city's health officials said.
Heping is currently preparing five of its 77 negative pressure isolation rooms, and if any H7N9 case is confirmed in Taiwan, all the others will be opened, the officials said.

The hospital's negative pressure isolation rooms — which are designed to prevent viruses from spreading outside — can accommodate 119 patients, the health officials said.

The city has heightened its alert following more H7N9 deaths reported in China, which has culled all poultry at a Shanghai market where the avian flu strain was found. Sales of live fowl in Shanghai have also been halted.

No H7N9 cases have been reported in Taiwan yet, but epidemic alert levels at all airports and seaports have been raised.
Taipei, which is a major destination for flights from China, has set up a cross-departmental task force to monitor the epidemic.

The officials said any travelers arriving at the city's Songshan Airport with suspected flu symptoms or fever will be sent directly to Heping for further observation.

Patients will first be screened at Heping's emergency unit and if they are found to be carrying flu viruses they will be sent to the negative pressure rooms via an exclusive elevator, the officials said.

Heping, which serves the city's Wanhua area, was locked down with all its patients and medial staff after a SARS epidemic broke out on its premises in 2003. A doctor and nurse contracted the flu and died during the outbreak.

The CNA said the hospital, which now serves about 2,000 outpatients daily, has since converted its seventh to ninth floors into a negative pressure isolation ward, which usually treats tuberculosis patients.

A head nurse, Yu Chin-mei, who was locked inside Heping to treat SARS patients in 2003, now faces the possibility of having to fight another deadly flu again.

Asked if she is afraid of H7N9, she was cited as saying: “Of course I'm afraid. But I'm a professional and therefore I need to understand H7N9 more than others.”

Yu said nurses are usually exposed to higher risks than doctors because they spend more time with patients.
The city's health officials said Heping has accumulated much disease-control experience from the SARS outbreak, and its facilities have also been upgraded in recent years.

The city is confident in Heping's capabilities, and other medical facilities of the city will also back up the hospital if the epidemic worsens, officials said.

After inspecting the city's disease-control facilities and equipment, Taipei's health director, Lin Chih-hung said stocks of surgical masks, protective gear, flu medicine and related items must be maintained at sufficient levels, and help from the central government will be sought in case of shortages.

Thermal scanners on standby at airports to anticipate H7N9 virus

A Health Ministry official said that the ministry had placed thermal scanners at all international airports in anticipation of an outbreak of the H7N9 virus, which recently hit mainland China as well as Taiwan.

“The equipment is ready, but there is an agreement with the World Health Organization (WHO) for countries in Southeast Asia to not activate the scanners just yet,” said Tjandra Yoga Aditama, director
general for disease control and environmental health at the Health Ministry, on Monday, as quoted by Antara news agency.

According to 2009 data, there are 27 international airports in the archipelago.

He said the scanners would be activated if the situation required the ministry to do so. He added that the ministry had also placed health officers at harbors as well as banners and posters.

“We have also checked on APD and oseltamivir drugs. Hospitals have been asked to report any suspicious cases of severe acute respiratory infections to our office immediately,” he said.

According to reports, the H7N9 virus has infected 126 people in China, claiming 23 lives. (asw/dic)

วันเดียวกัน ที่โรงแรมสุนีย์แกรนด์ฯ จ.อุบลราชธานี นพ.พรเทพ ศิริวนารังสรรค์ อธิบดีกรมควบคุมโรค (คร.) กล่าวภายหลังเป็นประธานเปิดงานมหกรรมรณรงค์ไข้เลือดออกภาคตะวันออกเฉียงเหนือ ว่า สถานการณ์โรคไข้เลือดออกทั่วประเทศ ตั้งแต่วันที่ 1 ม.ค.-30 เม.ย. 2556 พบผู้ป่วยโรคไข้เลือดออกจำนวน 26,067 ราย เสียชีวิต 33 ราย ยอดผู้ป่วยสะสมเพิ่มขึ้น 1,695 ราย เสียชีวิตเพิ่มขึ้น 5 รายจากสัปดาห์ที่ผ่านมา ได้แก่ ตาก 2 ราย สุโขทัย 2 ราย และภูเก็ต 1 ราย คร.ได้เร่งรัดและรณรงค์ให้ทำลายแหล่งเพาะพันธุ์ยุงและลูกน้ำก่อนเข้าฤดูฝน สำหรับพื้นที่ภาคตะวันออกเฉียงเหนือ ทั้ง 20 จังหวัด พบผู้ป่วยโรคไข้เลือดออก จำนวน 6,731 ราย มีผู้เสียชีวิต 7 ราย จังหวัดที่พบผู้ป่วยมากสุด 5 อันดับแรก ได้แก่ นครราชสีมา 921 ราย อุบลราชธานี 859 ราย ศรีสะเกษ 685 ราย สุรินทร์ 661 ราย และบุรีรัมย์ 526 ราย ตามลำดับ

– BEIJING – Health officials in China reported two new H7N9 infections, both from Fujian province, and four more deaths, boosting the outbreak’s total to 130 cases, 31 of them (24%) fatal.

One of the patients is a 9-year-old boy whose infection was detected during routine flu surveillance, according to official and media reports today. He has been discharged from the hospital, according to a statement Hong Kong’s Centre for Health Protection (CHP).

The other patient is a 69-year-old man who is hospitalized, according to a separate statement yesterday from the CHP. So far none of the man’s nine close contacts have shown any symptoms. China’s National Health and Family Commission today put the number of deaths at 31, an increase of four since the group’s last update, Xinhua, China’s state news agency, reported today.

The report did not include any other details about the deaths. The report also said 42 patients have recovered from their H7N9 infections. In other developments, China’s agriculture ministry yesterday announced five more poultry and market environmental samples that tested positive for H7N9, according to a report from the World Organization for Animal Health (OIE).

They included three environment samples from a live-bird market in Shandong province, one chicken sample from a live-bird wholesale market in Guangdong province, and one chicken sample from a market in Jiangxi province. Guangdong is the only province to report a positive bird or market sample in the absence of confirmed human H7N9 cases. It is also the southernmost area of China to report positive H7N9 findings.

The OIE report said 89,964 birds were destroyed at the Guangdong wholesale market, which is located in Dongguan. The health ministry said the H7N9 virus detected in the samples is similar to that found in a pigeon collected from a market in Shanghai in early April, Xinhua reported yesterday. Meanwhile, Tom Frieden, MD, director of the US Centers for Disease Control and Prevention (CDC) said the H7N9 virus in its current form can’t launch a pandemic, but it could if the virus mutates to gain the ability to spread readily from person to person, Reuters reported today. He added that predicting H7N9′s next step is impossible. “I cannot say with certainty whether that will happen tomorrow, within the next 10 years, or never.” Frieden told Reuters that the CDC has 193 staff members working on H7N9, with team members in China, Laos, Cambodia, and Vietnam.

Chinese tourists planning to visit Taiwan will have their visas stamped with a warning asking them to postpone their trip should they have a fever, according to Taiwan's immigration authorities.

The warning, reportedly written in red, will ask the potential visitors to postpone their travel plans if they have fever-like symptoms and also provides Taiwan's Center for Disease Control's 1922 emergency number should they fall ill once on the island.

Taiwan's immigration authorities have been closely monitoring the outbreak of the H7N9 avian flu strain across the strait, and has cooperated with the CDC on issuing visas to Chinese nationals. The authorities will also work together with Chinese travel agencies and tour guides to keep high alert on the tourists' health condition, and prevent an outbreak in Taiwan.

The virus has so far infected at least 128 people in China and killed 27, according to the latest data from China's health authorities on Sunday. The health of Taiwan's only H7N9 bird flu patient — a local man who contracted the virus on a recent business trip to eastern China — is said to be improving.

Ko also said the government will follow standard guidelines in responding to detections of the H7 or H5 avian flu viruses in local poultry.

First H7N9 case offers vital clues to fight flu

A Taiwanese businessman working in China, surnamed Lee (李), has been confirmed as the first H7N9 avian influenza case in Taiwan. The case has provided us with a lot of valuable information about how the H7N9 virus infects humans.

Lee stayed in Suzhou in China’s Jiangsu Province, for two weeks, was not in contact with birds or poultry and played golf once during the stay. This shows that in an H7N9-infected area, there is a constant risk of infection.

His falling ill after returning to Taiwan on April 9 shows that the virus’ incubation period is longer than that of a normal flu virus. During the early onset of the infection, he developed a light fever and experienced minor cold sweats and fatigue. During this period, a throat examination did not show any signs of the virus, which shows that a fever drives out none or only very little of the virus and thus will not infect others at this time.

The patient’s symptoms intensified rapidly and within a week of their appearance, the patient experienced lung and heart failure, and without extracorporeal membrane oxygenation it would have been difficult to save him. Only then was the virus discovered after examining a sputum specimen, which implies that the patient was beginning to secrete the virus and may have been able to infect someone coming into physical contact with him.

The H7N9 epidemic in China will not disappear in the near term and it could spread to southern provinces, such as Guangdong and Guangxi, and maybe even on to Hong Kong, as well as Taiwan. One can only hope that the disease prevention authorities have comprehensive response measures in place and that Taiwanese businesspeople and tourists traveling between Taiwan and China know how to protect themselves.

Questions about the source of H7N9, whether it spreads from person to person and how Lee contracted the virus during his two-week stay in Suzhou all remain unanswered. It has been two months since the virus first appeared in eastern China and it is now slowly spreading to the country’s northern and southern provinces. This means that the amount of people in the areas exposed to the virus is now in the hundreds of millions. Considering the virus’ infectiousness, one cannot help but wonder if the number of infected people is really just 120.

Given that the H7N9 virus is a composite of three different avian flu strains and that it has spread to an area covering hundreds of thousands of square kilometers in seven provinces, it appears that the source of the virus would be a poultry farm. There are close to 20 billion chickens in the affected areas, and these birds are required for the virus to grow enough so that it can infect humans.

Furthermore, the virus can be contained in poultry feces, which, when dry, become a powder that can be spread to remote places by the wind. Chicken droppings are rich in organic substances that can sustain the virus.

As for proof of whether the virus will spread between people, the Department of Health can test for that. Blood samples can be taken from people that visited the patient after he became seriously ill, such as family members, medical staff or friends who visited him within two to three weeks after they were tested for the virus. If the samples contain antibodies, that would be proof that the virus can spread from person to person, but that the excreted virus is not sufficient to cause infection or that the disease did not break out due to individual differences. If there were no antibodies, that would indicate that at the current stage, the H7N9 virus does not spread from person to person.
